At Mindful Movement Physical Therapy and Yoga, LLC, we respect your privacy rights. We have created this privacy policy to explain how we treat the information collected and received from your use of our digital platforms and anywhere you might connect with us, including in-person or over zoom or other digital audio/video connection platforms.  Except as set forth in this Privacy Policy and our Terms of Service agreement (“Terms of Service”) and other published guidelines, we do not release personal information about our users without their permission.

What Information this Privacy Policy Covers

By choosing to work with Mindful Movement, you may need to submit “Personally Identifiable Information” (i.e., information that can be used to identify you). Personally Identifiable Information can include information such as your name and email address,  as well as your location, among other things. You may also provide information regarding your health history. You are responsible for ensuring the accuracy of the Personally Identifiable Information you submit to Mindful Movement. Inaccurate information may affect our ability to provide services to you. For example, your email address should be kept current because that is one of the primary manners in which we communicate with you.

Except as set forth in this Privacy Policy or as specifically agreed to by you, Mindful Movement will not disclose any Personally Identifiable Information we gather from you on any of our platforms. At all times, Mindful Movement will comply with its obligations pursuant to the Health Insurance Portability and Accountability Act (HIPAA) and any information classified as protected under federal or State law with regard to medical treatment will be treated as private and confidential to the fullest extent of the law.  If the services provided to you by Mindful Movement constitute as medical treatment pursuant to the law, records related to that treatment will be so designated and appropriately segregated.

When users come interact with Mindful Movement, either in person or pursuant to a digital platform, we may track, collect and aggregate Non-Personal Information indicating, among other things, which content on our website was accessed, the order in which they were visited, when they were visited, and which hyperlinks were “clicked.” We also collect information from the URLs from which you linked to our website. Collecting such information may involve logging the IP address, operating system and browser software used by each user of the website. Essentially, cookies are a user’s identification card for the Mindful Movement servers. Beacons are small graphic files linked to our servers that allow us to track your use of our website and related functionalities. Cookies and Beacons allow Mindful Movement to serve you better and more efficiently, and to personalize your experience.  Cookies and beacons are not used to retain Personally Identifiable Information. Mindful Movement strongly believes in protecting the privacy of children. If you are under 13 years of age, then please do not use or access the platforms at any time or in any manner. We will take appropriate steps to delete any Personally Identifiable Information of persons less than 13 years of age that has been collected on our platforms without verified parental consent upon learning of the existence of such Personally Identifiable Information.
